PARTICIPATION FORM

Morgan County Heritage Day, October 15, 2022, 12:00-5:00 PM in the Village of
McConnelsville square. All booths must be ready by 12:00 PM.

Application deadline is August 15, 2022. The Heritage Day Committee reserves the right to
accept or reject applicants. Commercial products are not accepted. All participants are
responsible for their own tables, chairs, tents, and displays.

Payment may be made at the CVB office (Room 147, 155 E. Main St., McConnelsville), or mail check to: Morgan County Heritage Day, PO Box 599, McConnelsville, OH 43756
